Social Workers (Adults) – Transition Team | Birmingham City Council Up to £33.33/hr | Hybrid Working

Your new company

An exciting opportunity has opened up to join Birmingham City Council’s Transitions & Preparation for Adulthood Service as a Grade 5 Social Worker.

Your new role

We're looking for qualified Social Workers with 2+ years' post-qualification experience who are passionate about supporting young people (aged 17–25) with additional needs as they transition into adulthood. This includes working with individuals facing challenges such as homelessness, mental health issues, and involvement in the criminal justice system.

Key Responsibilities:
  • Conducting Conversation 3 discussions
  • Support planning & commissioning care (budget requests)
  • Reviews & mental capacity assessments
  • Attending multi-agency meetings (Best Interest, EHCP, Section 117)
  • Completing CHC checklists
You’ll be supported by experienced Senior Practitioners and a dedicated Team Manager.

Hybrid Working: 1 day/week at Sparkbrook Community Health Centre ⏱️
35 hrs/week | 3-month rolling contract
Up to Grade 5 – £33.33/hr

What you'll need to succeed

  • Recognised Social Work qualification (MA/BA/B.Sc., DipSW, CQSW)
  • Registration with Social Work England
  • Experience in adult services
  • Strong communication & teamwork skills
  • Flexibility to travel as needed
