A respected charity working to support people experiencing homelessness is seeking a Rough Sleeper Prevention Worker to join its assertive outreach team in Exeter. This role is vital in helping individuals move away from street homelessness and into safe, sustainable accommodation.

Your new role

  • Location: Exeter

  • Hours: Full-time, Tuesday to Saturday (including early mornings, evenings, and weekends)

  • Contract Type: Permanent

  • Deliver person-centred outreach support to rough sleepers across Exeter.
  • Conduct needs assessments, risk assessments, and develop support plans.
  • Facilitate access to short-term placements and provide transitional support.
  • Collaborate with healthcare professionals, local authorities, and partner agencies.
  • Participate in an outreach rota, including unsocial hours and Severe Weather Emergency Protocol (SWEP).
  • Maintain accurate client records and contribute to service development.

What you'll need to succeed

  • Experience working with homeless or socially excluded individuals.
  • Strong understanding of homelessness, offending, and social exclusion.
  • Excellent communication and interpersonal skills.
  • Ability to work flexibly and independently under pressure.
  • Competent in Microsoft Office and administrative tasks.
  • A full driving licence and access to a vehicle is essential.
Desirable:
  • Experience in multi-agency partnership working.
  • Knowledge of working with dual-diagnosis clients and complex needs.
