About the Role

This is a vital role supporting the delivery of quality assessments to ensure patients receive the care and support they need.

Role: Band 6 Continuing Healthcare (CHC) Assessment & Review Worker / CHC Assessor (there are 3 posts available)
Location: Remote (with some face-to-face assessments in Bath, North East Somerset, Swindon & Wiltshire if identified – the ICB will aim to limit these)
Hours/Shifts: Monday–Friday, 9am–5pm (full-time) Contract Type: temporary, starting ASAP until 31st December 2025 (might be extended subject to funding availability)

Requirements:

Demonstrable competence in completing CHC assessments and reviews.

Ability to work independently with strong attention to detail.

Flexibility to complete both remote and occasional face-to-face assessments.

Professional registration (e.g. NMP, HCPC)
