QSW - Adults - Social Worker - Hospital Team

Your new company

The team is based at the Royal Surrey County Hospital, one of Surrey’s five acute hospital teams, and includes both qualified and non-qualified staff. Besides the Royal Surrey Acute Hospital, the team also facilitates discharges from community rehab beds at Milford and Haslemere hospitals.
Patients receive support upon discharge through care packages designed to help them return to community living or short-term placements under the discharge to assessment framework. This framework allows for post-discharge assessments within a set timeframe, aiding patients in regaining their maximum independence.

Your new role

The role includes, but is not limited to, conducting Care Act assessments, Mental Capacity Assessments, screening residents for Continuing Healthcare, Carers assessments, Safeguarding work, and partnership working with NHS colleagues, including Discharge to Assess. The role also involves close collaboration with Adult Social Care Locality Teams and community health, charity, and voluntary organisations.
This position offers a fantastic opportunity to promote strengths-based and person-centred practices in supporting residents to be discharged from an acute hospital setting safely and promptly. It involves engaging the resident, their family, and carers in the process and working with partners to ensure care and support needs are met in a personalised manner.
The team provides robust support through regular supervision, management support, and peer-to-peer learning and support. The role involves working within a fantastic team that collaborates closely with health colleagues, fostering a positive and collaborative approach to the work.
The service operates from 8am to 8pm Monday to Friday and 9am to 5pm on Saturday and Sunday. Typically, the role requires working at least one weekend shift per month and one late shift per week. Staff scheduled to work a weekend shift will receive a day off in lieu. Full-time applicants will be contracted to a 36-hour week, with hours worked on a rota basis across 7 days a week. The role requires being onsite each working day. Further details regarding the working pattern can be discussed during the interview


What you'll need to succeed

Your application should include details about your:

  • Professional qualification in Social Work and current registration with Social Work England.
  • Safeguarding knowledge.
  • Knowledge and experience of current legislation, including the Care Act and the Mental Capacity Act, and its application in a social care setting.
  • Experience in conducting safeguarding enquiries, performing assessments within hospital and community settings, and working with a diverse group of individuals and partner agencies.
  • Experience in supporting the development of less experienced staff members.
  • Commitment to their own professional development.
  • Willingness to be based on site each day.
Surrey encompasses both urban and rural areas, and locality-based social care workers are expected to have a valid UK driving licence, access to a vehicle, and a willingness to travel across a wide geographical area.
