This service supports children and young people with autism and/or learning disabilities who are at risk of hospital admission or are currently in inpatient settings. The team operates in a high-pressure environment, managing extremely complex cases and responding to frequent escalations. Your new role

Role As a Project Worker , you’ll be a key practitioner delivering intensive support to children and young people with complex needs. You’ll work closely with families, education providers, health professionals, and social care teams to develop and implement tailored intervention plans. Your responsibilities will include:

  • Acting as a lead professional and coordinating multi-agency responses
  • Building trusted relationships with young people and their families
  • Supporting transitions from inpatient care to community-based settings
  • Advocating for the rights and needs of children and young people
  • Managing risk and safeguarding concerns with confidence and clarity
This is a full-time position (37 hours per week), requiring flexibility, resilience, and a deep commitment to improving outcomes for vulnerable individuals.


What you'll need to succeed

To thrive in this role, you’ll need:
  • Professional experience working with children and young people with autism and/or learning disabilities, especially in high-risk or inpatient contexts
  • Strong communication and coordination skills, with the ability to lead multi-agency planning and respond effectively to crisis situations
  • Emotional resilience and the ability to remain calm and focused under pressure
  • A solution-focused mindset, with a commitment to person-centred approaches
  • Relevant qualifications in social work, youth work, education, or health-related fields are highly desirable.
