Occupational Therapist Job based in Oxfordshire

Your new company

You’ll be joining the Homefirst Neighbourhood Team South, part of Oxfordshire’s wider Homefirst Hospital Service. This team plays a vital role in the newly implemented Discharge to Assess (D2A) process, supporting individuals as they transition from hospital to home. The service is focused on promoting independence and enabling people to regain control over their lives through timely, person-centred interventions.


Your new role

As an Occupational Therapist, you will be responsible for carrying out both bedside assessments in hospital settings and home assessments across South Oxfordshire. You will support individuals on their reablement journey, working closely with them to promote independence and improve their quality of life. Your work will include completing Care Act assessments, developing support plans, and collaborating with multidisciplinary teams to ensure safe and effective discharges. While you’ll be required to attend occasional meetings in the office, you’ll have the flexibility to complete paperwork from home.


What you'll need to succeed

To be considered for this role, you must be a qualified Occupational Therapist with at least two years of post-qualification experience. You’ll need access to your own transport, as travel across South Oxfordshire is essential for completing home visits. Experience in hospital discharge, reablement, and Care Act assessments will be key to your success, as well as confidence in working independently, with strong assessment and communication skills, and a commitment to person-centred care.
