We're looking to recruit a Housing First Practitioner in the Wirral on an ongoing, temporary basis to cover sickness cover. This is a 35 hour per week role (9-5 Mon to Fri) working 1 in every 6 weekends.

The successful candidate will hold a strong background in supporting vulnerable individuals facing multiple complex needs such as homelessness, rough sleeping, addiction and mental health.

Your new role

• Build trusting, professional relationships with individuals experiencing homelessness or barriers to permanent accommodation

• Deliver intensive, person-centred support plans based on individual strengths, goals and aspirations using trauma-informed practice

• Complete risk assessments, maintaining risk management and safeguarding throughout

• Provide practical support with benefits, housing options, budgeting, appointments and advocacy

• Work in partnership with multi-agencies to ensure holistic support

What you'll need to succeed

• Experience supporting individuals facing homelessness or rough sleeping, with an understanding of substance use and mental health

• Knowledge of welfare benefits, housing pathways and homelessness law

• A strong understanding of trauma-informed and strengths-based practice, with the ability to devise person-centred support plans

• Confident lone worker with the ability to manage a busy caseload with conflicting priorities
