Adult Social Worker Job in Birmingham

As part of the Discharge to Assess South Community Team, you’ll be working in a fast-paced, multi-disciplinary environment that plays a critical role in supporting individuals transitioning from hospital.


Your new role

We are seeking a qualified and experienced Social Worker to join the team, to support a service within the homeless and housing pathway for hospital discharges. You will carry out assessments and support planning using the Conversations Model. You will be working on complex cases which involve housing instability, substance misuse, safeguarding concerns, and mental health challenges. This role requires close collaboration with health professionals, housing officers, and voluntary sector partners.


What you'll need to succeed

  • A recognised Social Work qualification and Social Work England registration.

  • Experience in Adult Social Work, ideally within hospital discharge or community settings.

  • Strong knowledge of housing pathways, safeguarding procedures, and substance misuse support.

  • Ability to work effectively in a multi-disciplinary team.

  • Excellent communication, assessment, and planning skills.

  • A compassionate, resilient, and proactive approach to complex casework
