Fifteen years as a social worker/ care manager within a multi-disciplinary team in a 'complex needs' service, for people presenting with substance abuse (drug and alcohol) problems and, often, acute mental health problems, who may require urgent intervention. This included providing short-term intensive input.
Assess care needs and devising support plans/ care plans, supporting mental state examinations; implementation in collaboration with service users, carers/ 'significant others', the multi-disciplinary team and professionals from other statutory/ voluntary agencies.
Promoting personalisation and working to 'Best Practice' guidelines and the statutory / borough / organisational legislation, policies and procedures (Health & Community Care Act 1990, Mental Health Act 1983 – e.g. discharge planning under Section 117 and the Care Programme Approach), including those regarding substance misuse, substitute prescribing, mental health, physical health/ medication management, safeguarding children/ adults (i.e. domestic violence, intimidation) / parental support needed, developmental/ personal history, formal/ informal support networks, accommodation, financial matters/ welfare benefits, forensic history / outstanding criminal justice matters, education/ training/ employment, recreation/ leisure activities.
